How do you fish a purple worm?

The worm is still alive moving through the rocks on the bottom of the tank

why do i have anchor worm in my fish tank?? i clean it every 2 week and use a lot of good bacteria to break down food. so today 3 of my koi die and attach to it body there a alot of anchor worm. luckily i spot and save the 5 koi. im trying using a tweezers...
